Liability Waiver *

Assumption of Risk and Waiver of Liability I, the undersigned, understand that participation in the children’s program hosted by Light of Guidance at HIYCC involves various physical, creative, and group-based activities. These may include movement, arts and crafts, and interactive learning experiences which carry inherent risks, including minor injuries or accidents. I acknowledge that this event is voluntary, takes place outside of regular school hours, and is not a formal school activity. I understand that while volunteers and staff will be present to help supervise, I remain responsible for the safety and well-being of my child(ren) during participation. By submitting this form, I voluntarily assume all risks associated with this program for my child(ren), and I release and hold harmless Hadi Institute, Light of Guidance, HIYCC, their staff, volunteers, board members, and affiliates from any and all liability, claims, or causes of action arising from or related to injuries or incidents that may occur during the program, except in cases of gross negligence or willful misconduct. Responsibility for Medical Care I understand that Hadi Institute, Light of Guidance, and HIYCC are not responsible for providing medical treatment, first aid, or emergency services. In the event of a medical situation, I am responsible for any necessary care or costs for my child(ren). Behavioral Expectations I agree that my child(ren) will behave respectfully and follow the rules set by the program organizers and volunteers. I understand that any behavior that compromises the safety, structure, or enjoyment of the program may result in their removal from the event.
